รายงานการศึกษาเรื่อง “ระบาดวิทยาโรคจิตเวชร่วมในคนไทย: การศึกษาระดับชาติ 2551” นี้ เป็นส่วนหนึ่งจากผลการสำรวจระบาดวิทยาสุขภาพจิตและจิตเวชในคนไทยในปี พ.ศ.2551 ที่มีการดำเนินการต่อเนื่องทุก 5 ปี โดยกรมสุขภาพจิตกระทรวงสาธารณสุข เริ่มครั้งแรกในปี พ.ศ.2541 และครั้งที่ 2 ในปี พ.ศ.2546เพื่อศึกษาความชุกของโรคจิตเวชและปัญหาสุขภาพจิตของคนไทย แต่ที่ผ่านมายังไม่เคยมีการศึกษาถึงความชุกของโรคจิตเวชร่วมในระดับประเทศมาก่อน ข้อมูลโรคจิตเวชร่วมในคนไทย มีเพียงรายงานการศึกษาทางคลินิกเท่านั้น และต้องอ้างอิงจากข้อมูลการสำรวจในต่างประเทศ ซึ่งบริบทต่างจากประเทศไทยมากการศึกษาในประเด็นนี้ มีประโยชน์มากในทางการแพทย์และสาธารณสุขเนื่องจากหากมีการเจ็บป่วยทางจิตเวชหลายโรคในคนคนเดียวกันที่เรียกว่าโรคจิตเวชร่วม (psychiatric comorbidity) เช่น alcohol use disorders ร่วมกับmajor depressive disorder จะยิ่งทำให้อาการของโรคทั้งสองแย่ลง และกระบวนการบำบัดรักษาซับซ้อน ยุ่งยากมากขึ้น ต้องอาศัยการดูแลรักษาที่เข้มข้นมากกว่าและค่าใช้จ่ายในการรักษาเพิ่มสูงขึ้นในรายงานการศึกษาครั้งนี้ ไม่จำกัดเฉพาะโรคจิตเวชต่างกลุ่มวินิจฉัยโรค(เช่น โรควิตกกังวลร่วมกับโรคความผิดปกติทางอารมณ์) หรือโรคจิตเวชร่วมกับความผิดปกติพฤติกรรมดื่มสุราเท่านั้น ยังหมายรวมถึงโรคจิตเวชร่วมในกลุ่มโรคเดียวกันด้วย ซึ่งขอ้ มูลดังกล่าวจะมีประโยชนม์ ากทางคลินิก และที่สำคัญจะเป็นประโยชน์ในการวางแผนระบบบริการจิตเวชให้มีคุณภาพและประสิทธิภาพมากขึ้นทั้งในบริการจิตเวชตติยภูมิหรือการดูแลเฉพาะทาง และการเฝ้าระวังโรคจิตเวชร่วมในบริการปฐมภูมิและทุติยภูมิ รวมถึงเปน็ ขอ้ มูลพื้นฐานในการวางแผนคา่ ใชจ้ า่ ยสุขภาพกรณีโรคจิตเวชร่วมอีกด้วย

เป็นผู้จัดการแผนงาน เพื่อพัฒนาระบบ รูปแบบและวิธีการบำบัดรักษาผู้มีปัญหาการดื่มสุรา ที่เหมาะกับบริบทสังคมไทย โดยบูรณาการระบบ เข้าสู่ระบบบริการสุขภาพที่มีอยู่เดิม และพัฒนาเครือข่ายบริการทั้งในและนอกระบบสาธารณสุข ผ่านการมีส่วนร่วมดำเนินงานจากภาคีเครือข่ายทั้งในและนอกระบบสุขภาพ เพื่อช่วยกันขับเคลื่อนการดูแลแก้ไขปัญหาสุรา รวมทั้งสามารถเพิ่มช่องทางการเข้าถึงบริการของผู้มีปัญหาการดื่มสุราที่หลากหลายมากขึ้นการดำเนินงานของภาคีเครือข่ายได้รับการสนับสนุนจากแผนงานผรส. ทั้งในระบบบริการสุขภาพปกติ ตั้งแต่ระดับโรงพยาบาลส่งเสริมสุขภาพตำบล โรงพยาบาลชุมชน โรงพยาบาลศูนย์/โรงพยาบาลทั่วไป โรงพยาบาลจิตเวช โรงพยาบาลธัญญารักษ์และความร่วมมือจากระบบการบำบัดฟื้นฟูทางเลือกในชุมชนไม่ว่าจะเป็นวัด พระสงฆ์ องค์กรศาสนา เครือข่ายภาคประชาชนเครือข่ายผู้มีปัญหาการดื่มสุราที่สามารถเลิกสุราได้สำเร็จ แกนนำชุมชน องค์กรปกครองส่วนท้องถิ่น ตลอดจนเครือข่ายระบบยุติธรรมและระบบสถานประกอบการ ซึ่งช่วยการขับเคลื่อนสนับสนุนการดำเนินงานร่วมกันอย่างเป็นระบบ ทำให้การพัฒนาระบบ รูปแบบการดูแลช่วยเหลือผู้มีปัญหาการดื่มสุราเจริญก้าวหน้าและพัฒนาขึ้นตามลำดับตลอดระยะเวลาที่ผ่านมาครึ่งทศวรรษของแผนงานการ
พัฒนาระบบการดูแลผู้มีปัญหาการดื่มสุรา (ผรส.) ได้มีการศึกษาทั้งการทบทวนองค์ความรู้ การถอดบทเรียน การแปลเอกสารจากต่างประเทศ การวิจัยเพื่อพัฒนาระบบ รูปแบบการดูแลผู้มีปัญหาการดื่มสุรา แต่ยังมีการจัดการองค์ความรู้ (KM.) น้อย นับว่าเป็นโอกาสที่ดีที่ได้ร่วมเป็นส่วนหนึ่งภายใต้โครงการจัดการองค์ความรู้“ทศวรรษกลุ่มแผนงานแอลกอฮอล์” แผนงานผรส.ขอขอบคุณศูนย์วิจัยปัญหาสุรา (ศวส.) และบุคลากรของศวส.ทุกท่าน ที่ให้การสนับสนุนการจัดการองค์ความรู้ดังกล่าว ขอขอบคุณภาคีเครือข่ายทั้งในและนอกระบบสุขภาพทุกท่าน ที่เป็นกำลังสำคัญในการช่วยกันขับเคลื่อน พัฒนาระบบ รูปแบบการดูแลผู้มีปัญหาการดื่มสุราในสังคมไทยอย่างไรก็ตามความท้าทายในอนาคตที่มีการเปลี่ยนแปลงทั้งจากปัจจัยภายในและภายนอกประเทศ ซึ่งส่งผลกระทบต่อกระบวนการดูแลผู้มีปัญหาการดื่มสุรา จึงยังมีความจำเป็นที่จะต้องพัฒนาอย่างต่อเนื่องทั้งรูปแบบและวิธีการที่สามารถให้การช่วยเหลือผู้มีปัญหาการดื่มสุรา ในการเปลี่ยนพฤติกรรม เพื่อลด ละ เลิก การดื่มสุราต่อไป

วัยรุ่นในทุกวันนี้ ต้องเผชิญกับความเสี่ยงมากมายทั้งการใช้สารเสพติด ความรุนแรง HIV/เอดส์ การจัดการกับความเสี่ยงเหล่านี้ก่อนที่จะเกิดเป็นปัญหาอาจทำได้ค่อนข้างยาก เป้าหมายประการหนึ่งของ NationalInstitute of Drug of Drug Abuse (NIDA) คือการช่วยให้สังคมเข้าใจสาเหตุของการใช้สารเสพติดและป้องกันที่จุดเริ่มต้นของปัญหา การใช้สารเสพติดก่อให้เกิดผลกระทบร้ายแรงต่อครอบครัว โรงเรียน และชุมชนของเราหน่วยงาน NIDA ถือว่าพฤติกรรมการใช้สารเสพติดที่ผิดกฎหมายหรือแม้การใช้สารเสพติดที่ถูกกฎหมายในทางที่ผิด ล้วนถือว่าเป็นปัญหาการใช้สารเสพติดทั้งสิ้นในช่วงไม่กี่ปีมานี้ศาสตรด้านการป้องกันมีความก้าวหน้าไปปอย่างมาก วิธีการป้องกันหลายวิธีได้รับการทดสอบใน “สถานการณ์จริง” สามารถปรับใช้ในชุมชนได้ง่ายขึ้น

The Sixty-sixth World Health Assembly, consisting of Ministers of Health of 194 Member States, adopted the WHO’s Comprehensive Mental Health Action Plan 2013-2020 in May 2013. In 2019, the action plan was extended until 2030 by the Seventy-second World Health Assembly. Then in 2021, the Seventy-fourth World Health Assembly endorsed updates to the action plan, including updates to the plan’s options for implementation and indicators.
This updated Comprehensive Mental Health Action Plan 2013-2030 builds upon its predecessor and sets out clear actions for Member States, the WHO Secretariat and international, regional and national partners to promote mental health and well-being for all, to prevent mental health conditions for those at-risk and to achieve universal coverage for mental health services. While the updated action plan includes new and updated indicators and implementation options, the original four major objectives remain unchanged: more effective leadership and governance for mental health; the provision of comprehensive, integrated mental health and social care services in community-based settings; implementation of strategies for promotion and prevention; and strengthened information systems, evidence and research.
